The school nurse may administer medications to students during the school day with a written prescription from a physician, written permission from the parent/guardian, and the medication in the original or pharmacy-labeled container.
As of January 2021, only the approved NYS School Health Examination Form may be used to document health exams to provide consistency across the state. (See link to form below.)
Public Health Law- Section 2164: All Students enrolling in any school in New York State for the first time shall submit a written statement signed by a health care provider that they have been immunized as per NYS standards.
Students in grades 2,4, and 8 must be up to date with their immunizations. Students age 11 or older entering grade 6 must have a Tdap booster. All students entering grade 7 must have the meningococcal vaccine. These physical examinations and immunizations are due by 9/30/2022.
New York State law permits schools to request a dental examination in the following grades: school entry, K, 1, 3, 5, 7. Your child may have a dental check-up during this school year to assess his/her fitness to attend school.
